The Angel Tree Assistance Program is for those families who have children that are 12 years of age and younger and meet the eligibility guidelines Senior Angel Tree applications are also being accepted for those that are 65 years and older Applications will be taken IN PERSON ONLY. There will not be any online applications. To apply for Angel Tree Assistance: When: September 21-25, 2026 Time: 9a-12p & 1p-4p Location: The Salvation Army 2795 CR 220 Middleburg, FL 32068 Required Documents to bring: 1. Valid Photo ID 2. Proof of Current Residency with the Name of Parent/Guardian 3. Proof of Child Enrollment in school if applicable 4. Proof of Custody if applicable 5. Current Monthly Expenses - Please bring ALL Expenses 6. Current Mortgage Statement, Lease or letter from owner if living with others 7. Proof of ALL Income - 3 Pay stubs, SNAP/TANF Award Letter, 2025 Tax Return, Child Support, HUD Assistance, Proof of Loss of Income etc. 8. Valid Birth Certificates for each child If you do not have the required documents, you will need to return with the required information within the dates above before the application process can be started If you are unable to sign up during these dates/times due to a verifiable reason, please email us at salvationarmyclayctychristmas@gmail.com and we will strive to accommodate you with another time frame.
